User & Timestamp per Checklist item

A client requested to see per checklist item which user answered it and when. This becomes relevant for started-finished checklists or drafts uploaded to the server before final signature.

On the QA checklist details reviewer I would like to understand which user answered which items, especially if it has been filled by different users (e.g. on work started and finished).
